Ensure no persons working in Confined Space without Confined Space Entry Permit.

Assess site. Determine:

• Type of confined space (vessel, pit, tank etc)
• Geographical location of confined space. (It is vital to know the EXACT address/location of space for emergency response)
• Type entry/exit points
• Dimension of space, including entry/exit points
• Obstacles in and around confined space
• Type of work being done
• Number of persons entering confined space
• Known and potential hazards
• Lock out /tag out procedures required
• Communication systems in place
• Distance to medical treatment facilities
• Distance of nearest emergency assistance provider
• Expected response time of closest emergency assistance provider

Determine the type of emergency that could occur based on the type of confined space. Example:
