LOTO - Lock Out Tag Out Information:
The Lock Out Tag Out - LOTO Safe Work Method Statement (SWMS) is a comprehensive 10 page procedure.
This SWMS includes general guidance for Locking out and Tagging of plant and machinery to prevent plant and its components from activating (including release of stored energy) during inspection, maintenance, repair or cleaning. It also includes information for tagging plant that is in an unsafe condition.
Without procedures in place to ensure power is isolated to plant, persons can be killed or seriously injured from its accidental start-up.
The LOTO - Lock Out Tag Out procedure is ready to use and is not a blank template. Simply add your company details, ABN, and include any site specific details or risks and make it specific to the task at hand. It is then ready for printing and to be incorporated as part of your overall OHS System.
The LOTO - Lock Out Tag Out Work Method Statement can be used as part of your OHS inductions, for Tool box talks or tender applications.
Preparation
Ensure appropriate locking devices for type of plant. Examples:
- Padlocks
- Lockboxes
- Lock-out hasps (non-conductive, high visibility – if required)
- Electrical lockouts
- Cable lockouts
- Valve lockouts
- Safes and key cabinets
- Ensure suitable tags available:
- Meet requirements of AS 1319-1994, examples:
- Out of Service:
- Yellow background with bold black text "Out of Service" on each side
- Personal Danger:
- White background
- Recognisable danger symbol on each side
- Bold black text
- Durable
- 60 x 120mm
- Reliable means of attachment
Ensure only 1 key per lock. No duplicate keys.
Ensure ALL employees at site are trained in LOTO procedures
